Other  Confidence: High
UHealth, the University of Miami's academic health system, and UnitedHealthcare, the nation's largest insurer, still have no new contract, now just 5 days from a deadline that could take UHealth out of network.
The July 31 deadline is unchanged since we first covered this July 22. UHealth says United hasn't offered fair rates; United says UHealth wants outsized hikes. Medicare Advantage members keep coverage through August 31 under a separate deal.
Why it matters
If you or a family member is a UnitedHealthcare member in South Florida, watch for an out of network notice, though a deal could still land before July 31.

Enforcement  Confidence: High
The Trump administration is withholding $1.07 billion in federal Medicaid funding from California and Minnesota this month, citing suspected fraud in high-risk claims categories like in-home care.
California loses $867.5 million, Minnesota $199 million, spanning 14 flagged claims categories. Both states dispute the fraud framing and are weighing legal challenges. It's a deferral, not a permanent cut, but the cash is frozen now.
Why it matters
If you live in California or Minnesota and rely on Medicaid, especially in-home care, watch for payment or service disruptions while the review is underway.
