A federal judge just refused to pause new Medicaid work requirements days before millions of enrollees start needing to prove they're working, five months before the rule takes effect nationwide.
26 states and DC sued, arguing CMS illegally narrowed who counts as too medically frail to have to comply. The judge said the harm wasn't clearly CMS's fault. Requirements start January 1, 2027.

CMS just finalized $882.74 million more in Medicare payments to skilled nursing facilities for 2027, the long-term care homes that treat seniors recovering from hospital stays.
The 2.4% rate hike comes with a catch: every nursing home must now report the same patient-assessment data CMS uses to compute quality scores, even for residents who aren't on Medicare.
Why it matters
Medicare is paying skilled nursing facilities more next year, but the tradeoff is that every nursing home, not just Medicare-billing ones, now has to report detailed patient data faster.
