The Comprehensive Care for Joint Replacement Expanded model starts January 1, 2028 and covers most acute care hospitals. CMS projects the mandate will save Medicare $725 million total. Record A Henderson, Nevada physician was indicted for allegedly running a $95 million Medicare fraud scheme, billing for unneeded amniotic wound grafts on elderly and hospice patients to fund a lavish lifestyle. Prosecutors say Stephen Dubin, 74, took illegal kickbacks from allograft distributors through inflated invoices. Medicare paid out more than $54 million before the scheme was caught; he faces up to 10 years per count.
