Medicare just finalized a $2.1 billion hospital payment increase for 2027, and buried in the same rule made bundled payment mandatory nationwide for hip, knee and ankle replacement surgery for the first time ever.
The Comprehensive Care for Joint Replacement Expanded model starts January 1, 2028 and covers most acute care hospitals. CMS projects the mandate will save Medicare $725 million total.

A Henderson, Nevada physician was indicted for allegedly running a $95 million Medicare fraud scheme, billing for unneeded amniotic wound grafts on elderly and hospice patients to fund a lavish lifestyle.
Prosecutors say Stephen Dubin, 74, took illegal kickbacks from allograft distributors through inflated invoices. Medicare paid out more than $54 million before the scheme was caught; he faces up to 10 years per count.
