It self-reported the scheme in 2024\. Three insurers, Humana, UnitedHealthcare and Florida Blue, are now returning the extra payments to Medicare. No executive faced criminal charges. Why it matters This is the largest healthcare fraud settlement in Florida history and shows federal enforcers can still recover big money from Medicare Advantage coding schemes even after a company files for bankruptcy.
