Enforcement  Confidence: Medium
CMS says it has kept $1.6 billion out of fraudsters' hands in Medicare lab billing since January, using artificial intelligence to catch phantom test claims before providers ever get paid.
The tally includes revoking 157 fraudulent labs, saving $732M, and suspending 185 of roughly 600 flagged labs, blocking over $500M more in potentially fraudulent payments.
Why it matters
This is CMS touting its ability to catch fraud before it drains Medicare, and it's worth watching whether the AI tools flag legitimate labs by mistake.

Rule  Confidence: High
Connecticut's Attorney General, the state's top consumer watchdog, is asking regulators to reject 2027 health insurance rate hikes of up to 22.7% that would hit 220,000 residents.
Anthem wants 12.8% more for individual plans, ConnectiCare 22.7%, and UnitedHealthcare 18.9% for small groups. A final Insurance Department decision is expected in the coming weeks.
Why it matters
If regulators side with the insurers, 220,000 Connecticut residents could see premiums jump by double digits next year; watch the Insurance Department's decision in the coming weeks.
