Ablation still cut recurrence sharply. PVI-SHAM-AF (Lancet, ESC Congress) found AFEQT quality-of-life scores rose almost identically in both arms, not statistically different, despite ablation beating sham on AFib recurrence, 73% vs 52% free of AFib at 6 months. Why it matters This is the first sham-controlled trial to test whether AFib ablation's benefits go beyond keeping the heart rhythm steady, and it suggests some of what patients feel better from may be placebo. Watch whether the finding changes how doctors talk about the procedure, not whether it's still offered. ACC filed its comment letter on CMS's CY2027 hospital outpatient rule ahead of the August 31 deadline. The rule also proposes removing about 637 more procedures from Medicare's inpatient-only list; CMS finalizes in November. Why it matters This is a comment letter, not a final rule, so nothing changes yet, but it shows what cardiology's largest physician group is fighting for as CMS finalizes 2027 outpatient payment rates by November.
